Anesthesia in cesarean section


Abstract
In recent years, cesarean section is being done with increasing frequency because, properly executed, it carries little more risk for the mother and often less risk for the infant than vaginal delivery.
In past years, most maternal deaths were caused by eclampsia, bleeding, infection and blood transfusion reaction. Obstetric management has been improved by better blood cross-matching and antibiotics, but difficulties remain.
Optimal results require effective teamwork among the obstetrician, anesthetist and pediatrician. They should be aware of the many problems which are related to maternal physiology, and pharmacology, anesthesiology, and fetal complications.
The choice of anesthesia for cesarean section is controversial. Ideal anesthesia should 1) produce efficient relief from pain 2) not depress the respiration of the fetus 3) not depress the uterus 4) be non-toxic 5) be safer for mother and child, preventing airway obstruction by vomitus.
The author has discussed maternal physiology and pharmacology, placental physiology, anectheciology and fetal complications.
